New Collision Reconstruction Assignment Form


Instructions

To make assignment submission easier, we have created a form for electronic submission.  The form is available in two formats.  The first file is in the Microsoft Word Document (.doc) format.  If you have Microsoft Word, you can download this file and save it to a convenient location (such as your Desktop) for repeated use. Simply double-click on the file to open the form, fill out the required fields, and then forward a copy of the completed form to mrochon@rochons.ca.  After filling out the form, forwarding of the completed form by email can be initiated from Microsoft Word by selecting File > Send To > Mail Recipient (As Attachment)...  Additionally, you can save the file for your records by selecting File > Save As.  This will save the file in the Word Document (.doc) format without saving over the original.  If you want to ensure that you don't accidentally save over the original, you may right click on the file, go to "Properties", check off "Read Only" and say "OK".  If you experience any problems emailing the form from Microsoft Word, you can save the file normally (as above) and then manually attach it to an email to Mr. Rochon (mrochon@rochons.ca).  You can also print the completed form and fax it to us if you prefer (Fax: 905-660-9994).

If you do not own Microsoft Word, you may download the OpenOffice.org software suite--which opens Microsoft file formats--for free from www.openoffice.org, or you can download the following PDF version of our form, print it, fill it out by hand, and then fax it to our office (Fax: 905-660-9994).
